A váradhegyfoki káptalan előtt Corvin János özvegyének, Beatrixnek, Frangepán Bernoldus gróf leányának a megbízottja: Werhyke-i Chwbranych Miklós úrnője megbízásából bemutatta IV. Béla királynak 1242. nov. 21-én kelt, Dénes Szlavonia hercege és bánja ugyancsak 1242. évi átíró oklevelét és kérte annak átírását, amit a káptalan teljesített is. - Hátlapjára nyomott pecsét nyomaival. – Regeszta forrása: OL regeszta.
