De Capistrano János ferencrendi szerzetes, heretice pravitatis generalis inquisitor, Balahth-i Pálhoz, feleségéhez: Margithoz, fiaihoz: Miklóshoz és Tamáshoz, leányaihoz: Erzsébethez, Katalinhoz és Affrához. Mivel a vett anyagi támogatásért nem tud anyagi ellenszolgáltatást nyujtani, felveszi őket a rend confraternitásába és részesíti őket mindazokban a lelki kiváltságokban, amelyek a rend tagjait illetik életükben és halálukban. – Függő pecsétje hiányzik.
