Parasceves. Kerezthwr-i Miklós és Benedek tanúsítja, hogy Bathyan-i Boldizsár, a néhai Dombo-i Dávid végrendeletének végrehajtója, visszaadta a Kerezthwr-i egyház viaticumát, amelyet a néhai Dombo-i Dávid vitt el az egyháztól. Nyugtatják Boldizsárt, továbbá a végrendelet többi végrehajtóját, s Dombo-i Dávid rokonait e tekintetben. Eredeti, papír, az oklevél szövege alá nyomott két papírfelzetes gyűrűspecsét, a második ép.
