A csázmai káptalan előtt Mátyás (?) plébános, Gradewcz-i Banych, Zwybowcz-i Pál, Fazekas Gergely providi, az Othnya-i Szent Pál egyház vitricus-ai vallják, hogy Batthyan-i Boldizsár és Orsolya, Gereben-i Herman (fia) László özvegye a 100 arany forintról, amelyet László végrendeletileg a Szent Pál egyház helyreállítására hagyott, eleget tettek. Nyugtatják Boldizsárt, Orsolyát, Boldizsár fiait: Boldizsárt, Ferencet és Bertalant s Mykulasych Miklóst az összeg tekintetében. – A hátoldalon rányomott pecsét nyomai. – Regeszta forrása: OL regeszta (Bándi).
