A leleszi konvent előtt Anna asszony, Csapi Zsigmond özvegye, nyugtatta a konvent előtt Csapi Györgyöt, miután ez neki elhalt férjének az Ung megyei Chap, Zthrythe, Rath és Assowan, a Zemplén megyei Galzech, Polyanka, Zabocz, Thehnya és a Bereg megyei Som, Sapzom és Chethfalwa possessiókban bírt részei után őt megillető hitbér és nászajándék fejében pénzbeli fizetéssel teljesen eleget tett. - Papír, utolsó 4 sorában elhalványult írás - Hátlapján rány. p.töred. – Regeszta forrása: OL regeszta (Ila).
