A kolosmonostori konvent jelenti Herepe-i Márk erdélyi alvajdának, hogy megkapta vizsgálatot elrendelő oklevelét és annak értelmében Zylwas-i Balással mint vajdai emberrel kiküldte testimoniumként religiosum virum fratrem Augustinum sacerdotem, akik feria quinta prox. a. f. b. Dionisii mart. (okt. 8.) Torda megyében körüljárva és mind a nemeseknél, mind a nem nemeseknél érdeklődve, megtudták, hogy minden a jelentésnek megfelelően történt. Hátlapján pecsét nyomai.
