9. duben 1304, Praha
Jan, biskup pražský, potvrzuje inserovanou listinu z 31. srpna 1288 (Vratislav), jíž Tomáš, biskup vratislavský, konfirmuje inserovanou listinu z 2. srpna 1288 (Jelenia Góra - Hyrzberch), kterou Bolek, kníže slezský a pán ve Lwówku (Lewenberch), dal johanitskému špitálu svatého Jana Křtitele podací kostela ve vsi Chmielno (Rusch),1 a ustanovuje, aby tamní plebán Jan spravoval kostel dále bez překážky ze strany johanitů.
