A kapornaki konvent II. Ulászló király 1503. augusztus 22-i parancsára Gerse-i Pethew Jánost Vasvárott, Sye-i Loranth Mihályt pedig Oladon megidézte a personális presentia elé, minthogy ezek nem voltak hajlandók a vasvári prépostnak és káptalannak visszaadni a Septhey Péter által rájuk hagyott jogbiztosító iratokat. – A hátlapon feljegyzés Loranth Mihály Szent Márton napja előtti csütörtökön való megjelenéséről. – A hajtások mentén nagyobb darabok kiszakadtak.
