A somogyi konvent Hédervári Lőrinc nádornak. 1446 június 19-i parancsára Gimesi Forgách Benedek és Herbartya-i Osth János felesége: Anna asszony a Somogy megyei Mochola és Mernye possessiókban elzálogosított részek birtokába a név szerint felsorolt szomszédok jelenlétében Pernez-i Pétert ellenmondás nélkül bevezették. – Bal alsó sarkának utolsó 4 sorából 3-4 szónyi rész hiányzik. – Hátlapján rányomott pecsét maradványa.
