Az augusztus 1. után tartott tolnai megyegyűlésen Mes-i István Nagmyhal-i Albert vránai perjel Fad-i officialisa eltiltja a székesfehérvári káptalan Fad-i és Tholnawar-i jobbágyait a keresztesek Fad-i birtokán lévő makkos használatától. Eredeti, papír, csonka, csaknem harmada hiányzik, hátlapján gyűrűspecsétek nyoma.
