Capistranum-i János a ferencrendiek hegyekentúli generalisának vikariusa a ferencrendi minorita szerzetesek, a minorita apácák, a Szent Klára és szent Damján rend részéről kapott felhatalmazás alapján, mint rendjének jótevőit a würzburgi egyházmegyei Munsteini Kog (Koch) studentem Wmensem, atyját, anyját, testvéreit és összes rokonságát felveszi a rend confraternitásába és a rend által elért kegyelmek részesévé teszi. - Függőpecsétje hiányzik.
