A csázmai káptalan előtt Monozlo-i Chupor Pál bán fia György a maga és testvére, Demeter, zágrábi püspök, továbbá Chupor Ákos fia Miklós nevében a körösmegyei Felsew és Alsow Kozowcz, Maryassowcz, Polachno nevű, most nagyobbrészt elhagyott falvait a ”sub monte Garigh” levő páloskolostornak adományozza. - Hártyán, vörös-fekete selyemről függő kopott pecséttel. - Regeszta forrása: Publikált regeszta (Mályusz Elemér): LK 12 (1934) 115/321.
