(Bartholomei ap.), Buda. Perényi Imre nádor királyi helytartó a pálosok Laad-i Szűz Mária kolostorának kérésére és bemutatásában átírja a néhai Korneth-i Mihály nemes, Jazo-i várnagy saját pecséttel megerősített 1508. április 28-án végrendeletét, (BLÉ V [1985] 666), a Borsod megyei Ewrews birtokról és az abban foglaltakhoz hozzájárul. - Hártyán, rózsaszín-sárga-zöld zsinóron ép függőpecséttel, ad relacionem domini Stephani Keserew vicepalatini kancelláriai jegyzettel az oklevél jobb alsó sarkában. - MOL, (AP Lád f.12 n.2) - Regeszta forrása: Publikált regeszta (Bándi Zsuzsanna): BLÉ 5 (1985) 666.
