(X.Kal.Novembris). Zsigmond király György pálos rendfőnöknek a maga és a rend Laad-i Szűz Mária kolostorának kérésére és a rendfőnök bemutatásában átírja és megerősíti Olnod-i Zudar (dictus) Péter bán fiai: Jakab és Simon oklevelét a Borsod megyei Rakushaza birtokról s az egri káptalan 1414. július 25-én kelt, a beiktatásról szóló jelentést (BLÉ V [1985] 629). - Restaurált hártyán, alul függőpecsétre utaló bevágásokkal, zöld pecsétzsinór maradvánnyal.
