Zalavár, postera Joann. Bapt. ex lacunis diui Adriani martiris de Zala. Nagy fájdalmára megbízatásának nem tudott eleget tenni. Reméli azonban, hogy néhány nap múlva teljesíti azt. Eredeti, papír, szakadozott, Záró gyűrűspecsét nyoma. Az aláírás: ”seulus invtilis Beati Adriani martisis.” II. Ulászló király 1511. július 31-én, a zalai konventhez intézett mandátuma, bár a notarius utólag kihúzta Gersse-i Tamás (Így!
