Bude. feria quinta proxima post festum passce Domini. Lajos király (rex Hungarie, Polonie, Dalmatie etc.) jelenti, hogy Ardo-i Zouard fia: János a saját és az összes Ugocha megyei nemesek nevében bemutatta előtte Erzsébet királynénak 1355. augusztus 29-én kelt teljes egészében közölt oklevelét, kérve annak megerősítését és átírását, amit a király teljesít is.
