    Ego Conradus dictus Hake plebanus ecclesie parochialis in Welsa Pataviensis diocesis universis et singulis quorum interest vel intererit cupio enodare, quod famosus vir Hertwicus dictus Kremer opidanus ibidem in Welsa in laudem omnipotentis Dei et gloriose virginis matris ejus Marie necnon sancte Elizabeth ob remedium anime sue salutare ac omnium suorum progenitorum altare situm in capella hospitalis beate Elizabeth opidi prefati fundavit et erexit ac certis redditibus dotavit nostro pleno consensu et assensu, cujus quidem altaris jus presentandi quotiens vacare contingerit ex benivola mea voluntate sibi et suis heredibus nec non omnibus eorundem in sua geneoliga (sic) successoribus perpetuis temporibus reservavit ita, quod rector altaris predicti pro tempore existens contra oblationes necnon ceteros quoscunque questus ac testamenta dicte mee ecclesie nichil penitus debeat usurpare, sed idem rector ipsius altaris pro tempore poterit et debebit temporibus exigentibus cum sacramentis ecclesiasticis infirmos dumtaxat in dicto hospitali degentes curare et expedire. In quorum evidentiam presentem litteram feci cum appensione sigilli mei firmiter roborari. Datum anno domini M°cccl xx quarto (millesimo trecentesimo septuagesimo quarto) sedecima die mensis Maij.
